How to submit your SplitFit expenses to Harvard Pilgrim for reimbursement

Harvard Pilgrim Health Care (HPHC) reimburses for SplitFit sessions as part of their fitness reimbursement program.

NOTE: For the reimbursement forms, please use this address for SplitFit (even if you use multiple locations):  SplitFit, 1285 Beacon Street, Brookline, MA 02446

For your statement of SplitFit sessions and expense simply e-mail support@splitfit.com with your request including time period.
